all right welcome back everyone to
  • 00:00:02
plant-based Kidney Health as always I'm
  • 00:00:04
Dr Sean Hashmi my partner is Michelle
  • 00:00:06
crossmer Michelle we are back for
  • 00:00:09
another episode and I gotta tell you I
  • 00:00:11
was just looking at the numbers this is
  • 00:00:13
    our episode number 104.
  • 00:00:19
    that's a lot
  • 00:00:20
for those of you guys who are joining us
  • 00:00:22
for the first time you got 103 more
  • 00:00:25
episodes to watch to learn all about
  • 00:00:26
kidney disease and what's fascinating is
  • 00:00:29
is after doing over a hundred episodes
  • 00:00:31
we're still getting questions and we're
  • 00:00:35
still realizing there's so many topics
  • 00:00:36
we haven't even covered yet yeah it's
  • 00:00:39
crazy when we you know look at the
  • 00:00:40
comments and suggestions on episodes I
  • 00:00:42
mean at the uh Google list that I have
  • 00:00:45
where you know I keep everything
  • 00:00:48
there's like hundreds and hundreds of
  • 00:00:50
topics on there so again we're keeping
  • 00:00:53
track of them we try to do these as
  • 00:00:55
often as we can but definitely there's
  • 00:00:57
some that come up and we're are asked
  • 00:00:59
multiple times over and over again so
  • 00:01:01
and then we like to address them so
  • 00:01:04
all right so with that guys today's
  • 00:01:06
topic is all about metabolic acidosis
  • 00:01:09
what the heck is it and why should you
  • 00:01:10
care so let's start by telling you a
  • 00:01:13
little bit about what happens in your
  • 00:01:15
body and why the kidneys are so vital to
  • 00:01:17
what happens you know when you have your
  • 00:01:20
    kidneys that are functioning normally
  • 00:01:21
they do this remarkable job of balancing
  • 00:01:25
the acid and the base in the body if you
  • 00:01:27
get too much of Base in the body or too
  • 00:01:30
much of acid in the body they're the
  • 00:01:32
ones who regulate it now your breathing
  • 00:01:34
is something that can do it short term
  • 00:01:36
so in other words if you have too much
  • 00:01:39
acid in the body what's going to happen
  • 00:01:41
is you're going to start to breathe
  • 00:01:42
faster because you want to blow off the
  • 00:01:45
acid from your lungs out there and the
  • 00:01:48
idea there is you're creating an
  • 00:01:50
environment with more bass inside your
  • 00:01:53
body to counteract the acid but that's a
  • 00:01:56
very limited amount of work you can do
  • 00:01:58
and that's short term long term is the
  • 00:02:01
job of the kidneys so when we talk about
  • 00:02:03
acidosis acid or too much acid is
  • 00:02:07
actually an independent risk factor for
  • 00:02:10
the progression of kidney disease so
  • 00:02:12
here's where it becomes interesting as
  • 00:02:14
kidneys get worse you start to get more
  • 00:02:16
acid buildup inside the body as more
  • 00:02:19
acid builds up inside the body it makes
  • 00:02:21
the kidneys worse so you gotta
  • 00:02:24
understand this Loop because too many
  • 00:02:27
    people only think it's a one-way street
  • 00:02:29
but it's actually both ways and so the
  • 00:02:33
reason we want to be concerned about it
  • 00:02:35
is because when you start to have it now
  • 00:02:37
symptoms wise you're going to have all
  • 00:02:39
sorts of symptoms acutely so you might
  • 00:02:41
see that your heart rate is beating
  • 00:02:42
faster wide because you're trying to
  • 00:02:44
breathe faster you may feel really weak
  • 00:02:46
or really tired you might even have
  • 00:02:48
headaches or confusion because of the
  • 00:02:50
acid that's starting to build up and
  • 00:02:52
some people will come back and say
  • 00:02:53
there's a loss of appetite so in
  • 00:02:55
    advanced kidney disease acidosis can
  • 00:02:58
    cause
  • 00:02:59
loss of appetite but so can the buildup
  • 00:03:02
of toxins and how do you know which one
  • 00:03:04
is which we have treatment so we'll get
  • 00:03:06
to in a second as far as that goes but
  • 00:03:09
it's not just the short-term effects
  • 00:03:11
it's the long-term effects so what are
  • 00:03:13
the long-term effects of acid so acid as
  • 00:03:16
you know if I was to pour acid inside
  • 00:03:18
you know Michelle's house she would kill
  • 00:03:20
me first of all but let's say I got away
  • 00:03:22
with that and I poured it on the floor
  • 00:03:24
what does it do to the floor it eats
  • 00:03:26
away at the floor now I imagine that
  • 00:03:28
extra acid inside your body what does it
  • 00:03:31
    do to your bones
  • 00:03:32
it causes osteoporosis it leeches out
  • 00:03:35
all the minerals from your bones so
  • 00:03:37
metabolic acidosis will lead to a much
  • 00:03:40
higher risk that you're going to break
  • 00:03:42
your bones get fractured when we talk
  • 00:03:44
about kids for example when kids have
  • 00:03:47
kidney disease what happens if we leave
  • 00:03:49
the acidosis there they can't grow to be
  • 00:03:52
their full potential so in other words
  • 00:03:55
it can prevent the release of growth
  • 00:03:57
hormone in young people and growth
  • 00:04:00
hormone in young people is needed for
  • 00:04:03
proper growth now kidney disease the
  • 00:04:05
challenge that you have is the more acid
  • 00:04:07
you have the more it's going to cause
  • 00:04:09
that kidney function to start to decline
  • 00:04:11
faster and faster and faster and it's
  • 00:04:14
not limited to Bones it's not limited to
  • 00:04:16
kidneys it also starts to cause muscle
  • 00:04:19
loss so this is really fascinating
  • 00:04:21
because excess acid will eat away at the
  • 00:04:24
muscles and the muscles will start to
  • 00:04:26
break down and if you've seen folks who
  • 00:04:28
are older we talk about muscle wasting
  • 00:04:30
well in kidney disease it's the same
  • 00:04:33
concept that there's muscle wasting and
  • 00:04:36
then the other part of this that ends up
  • 00:04:38
happening is this acid or acidic
  • 00:04:41
environments they can happen for couple
  • 00:04:44
of things so for example if you're a
  • 00:04:46
diabetic and you have a lot more
  • 00:04:48
metabolic which is the body acidosis you
  • 00:04:52
will also find that you have insulin
  • 00:04:54
resistance and so this makes it
  • 00:04:56
difficult because blood sugar levels are
  • 00:04:58
going to run higher and if you watch one
  • 00:05:01
of her previous episodes where we talked
  • 00:05:02
about sugar and how sugar is causally
  • 00:05:05
meaning cause and effect is causally
  • 00:05:07
linked to kidney disease now you have an
  • 00:05:10
environment with more acid leading to
  • 00:05:12
more insulin resistance leading to more
  • 00:05:14
sugar which is leading to kidneys
  • 00:05:17
getting worse and then the final thing
  • 00:05:19
you want to know about this is what is
  • 00:05:22
the ultimate thing that happens it leads
  • 00:05:24
to early death and so as a result of all
  • 00:05:27
of this there's lots of treatments now
  • 00:05:29
Michelle's going to talk about what the
  • 00:05:31
food options are but we also have pills
  • 00:05:34
and those pill options are really things
  • 00:05:36
like sodium bicarbonate we don't really
  • 00:05:39
    use potassium bicarbonate that much
  • 00:05:41
    because kidney patients potassium is
  • 00:05:43
already high but it's sodium bicarbonate
  • 00:05:44
the other thing is we use sodium citrate
  • 00:05:47
citrate is just a precursor meaning it
  • 00:05:50
converts into bicarbonate but keep in
  • 00:05:54
mind the word sodium is in all of the
  • 00:05:57
things we're mentioning so we'll talk
  • 00:05:59
about all the wonderful negative things
  • 00:06:01
that the word sodium comes with but
  • 00:06:03
before we do that as we're talking about
  • 00:06:06
    options on how to correct
  • 00:06:08
    metabolic acidosis Michelle when we
  • 00:06:12
think about this what about fruits and
  • 00:06:14
veggies you know as we talk about this
  • 00:06:16
stuff can fruits and veggies sort of
  • 00:06:18
step in and do stuff kind of like what
  • 00:06:20
    sodium bicarbonate can do
  • 00:06:23
yeah so the capacity of any food to
  • 00:06:26
produce either acid or Alkali is called
  • 00:06:29
the potential renal acid load and we've
  • 00:06:31
talked about this in other episodes
  • 00:06:33
um but protein-rich Foods produce more
  • 00:06:36
acid and fruits and vegetables produce
  • 00:06:39
more Alkali or more face and so they've
  • 00:06:41
    actually
  • 00:06:43
um there was a study that they looked at
  • 00:06:44
it was a five-year study but they looked
  • 00:06:46
    at non-diabetic kidney disease patients
  • 00:06:49
that had acidosis and they were assigned
  • 00:06:52
either to sodium bicarbonate fruits and
  • 00:06:54
veggies or just their usual care and so
  • 00:06:57
after the five years what they found is
  • 00:06:59
that both the sodium bicarbonate group
  • 00:07:02
so the people prescribe that medication
  • 00:07:04
and the fruit and veggie group both
  • 00:07:06
raised their serum bicarbonate levels so
  • 00:07:10
it helped to treat the metabolic
  • 00:07:12
acidosis but the people who were in the
  • 00:07:14
    fruit and veggie group had
  • 00:07:17
um better cardiovascular markers so they
  • 00:07:19
had lower systolic blood pressure and so
  • 00:07:22
it just goes to show not that there is
  • 00:07:24
not that the sodium bicarbonate it's not
  • 00:07:25
something that's used and wrong and that
  • 00:07:27
you shouldn't be doing but everyone can
  • 00:07:29
and should be working to have more
  • 00:07:31
fruits and veggies in their diet because
  • 00:07:33
of either the capacity to help treat
  • 00:07:35
metabolic acidosis or to help prevent
  • 00:07:38
    metabolic metabolic acidosis in people
  • 00:07:40
    with kidney disease so
  • 00:07:43
um so knowing that I mean again everyone
  • 00:07:45
that's right we're so like fruits and
  • 00:07:46
veggies fruits and veggies but sodium
  • 00:07:48
bicarbonate can you explain then what is
  • 00:07:51
that right how does that work and then
  • 00:07:55
um I well then we'll get into kind of
  • 00:07:57
that the household remedy that people
  • 00:07:59
want to try to just drink in place of it
  • 00:08:02
    basically sodium bicarbonate is nothing
  • 00:08:05
more than Salt and baking soda that's
  • 00:08:07
probably the simplest explanation and it
  • 00:08:10
comes as pills which is kind of hard to
  • 00:08:12
swallow but it comes as pills it comes
  • 00:08:14
as liquid and you can take both of them
  • 00:08:17
    if that's something that your
  • 00:08:18
nephrologist is asking but you have to
  • 00:08:21
understand what is the effects but more
  • 00:08:24
importantly what is the side effects of
  • 00:08:27
what's going to happen when you end up
  • 00:08:28
    taking these
  • 00:08:29
so can someone then like let's just say
  • 00:08:31
someone has kidney disease and they
  • 00:08:34
don't want to eat fruits and veggies can
  • 00:08:35
they just drink baking soda yeah it's a
  • 00:08:38
terrible terrible idea so hear me out
  • 00:08:41
whenever you you hear somebody on the
  • 00:08:43
internet trying to tell you like you
  • 00:08:45
know take this concoction and it's going
  • 00:08:47
to fix your kidney disease please don't
  • 00:08:48
unless you fully understand it you know
  • 00:08:51
it's it's um in my world right now with
  • 00:08:54
    weight loss medications everybody wants
  • 00:08:56
    Rosenberg
  • 00:08:57
Etc it's the same exact thing is nobody
  • 00:09:00
wants to understand the full scope so I
  • 00:09:02
said the most important thing in sodium
  • 00:09:04
bicarbonate is the sodium part and why
  • 00:09:07
should you care because as you start to
  • 00:09:09
put too much salt if you put too much
  • 00:09:11
salt and your body can't manage the
  • 00:09:14
levels inside your blood you can
  • 00:09:16
actually get seizures from it and so too
  • 00:09:18
little salt can give you seizures coma
  • 00:09:20
and death too much salt can give you
  • 00:09:23
seizures coma and death so this is not a
  • 00:09:26
minor thing you can get nausea and
  • 00:09:27
vomiting from it you can get muscle pain
  • 00:09:30
from it you can have stuff like where
  • 00:09:32
you have to pee all the time you can
  • 00:09:34
start to build up fluid if you have any
  • 00:09:36
heart issues and the heart can't pump
  • 00:09:38
well enough the salt is like a magnet
  • 00:09:41
for water so it's holding on to water
  • 00:09:43
now you have all this extra volume that
  • 00:09:46
your heart has to try to squeeze if it
  • 00:09:48
can't the blood will back up work it
  • 00:09:50
will back up into your lungs we call
  • 00:09:52
that pulmonary edema and on top of all
  • 00:09:55
of these things the bicarbonate portion
  • 00:09:58
if you're not under the guidance of a
  • 00:10:00
    healthcare practitioner who's monitoring
  • 00:10:03
your blood work you can go from the
  • 00:10:04
acidosa side too much too much of the
  • 00:10:08
alkalosa side and once again seizures
  • 00:10:11
comment that from the salt component but
  • 00:10:13
    metabolic alkalosis in itself can
  • 00:10:15
actually cause your heart to slow down
  • 00:10:17
it can cause you to slow down your
  • 00:10:19
breathing going on because the concept
  • 00:10:21
there is is if your alkalosis meaning
  • 00:10:25
too much alkaline in your body what does
  • 00:10:28
your lungs try to do counteract that so
  • 00:10:31
let's think about this for a second when
  • 00:10:33
there's too much acid we said the lungs
  • 00:10:36
breathe faster to blow off the acid in
  • 00:10:39
the lungs if there's too much alkaline
  • 00:10:42
in the body the lungs don't want to
  • 00:10:45
breathe fast they want to slow down
  • 00:10:47
their breathing so that they can hold on
  • 00:10:49
to the acid to counteract the alkaline
  • 00:10:53
so what happens breathing can become
  • 00:10:55
incredibly slow so this is why it's not
  • 00:10:58
a simple answer is why don't you just
  • 00:11:00
take that step a much better answer is
  • 00:11:03
the one that Michelle just talked about
  • 00:11:04
which is why don't you eat more fruits
  • 00:11:07
and vegetables the data shows that
  • 00:11:09
you're going to get exactly the same
  • 00:11:11
    benefits from a diet that's
  • 00:11:13
    predominantly
  • 00:11:15
plant-based as you are we're taking salt
  • 00:11:17
and baking soda so if that's the case
  • 00:11:19
and then you're going to get the
  • 00:11:21
antioxidants you're going to get the
  • 00:11:22
fiber you're going to get the water
  • 00:11:24
    that's packaged perfectly inside the
  • 00:11:26
fruit and vegetables why the heck
  • 00:11:29
wouldn't you take Nature's best medicine
  • 00:11:31
instead of just me giving you some salt
  • 00:11:34
    and baking soda
  • 00:11:36
    yep great question and
  • 00:11:39
um and again we're not saying if you're
  • 00:11:40
prescribed that like not to do that by
  • 00:11:42
any means but it's where again
  • 00:11:44
    prevention and being proactive are
  • 00:11:46
things that were always we're always
  • 00:11:48
preaching so hopefully that answers your
  • 00:11:50
guys question a little bit on metabolic
  • 00:11:52
    acidosis uh so
  • 00:11:55
um sodium bicarbonate baking soda and
  • 00:11:57
eating more fruits and veggies we'll see
  • 00:11:59
you guys next time thanks guys
