摘要

TLDRIn a pivotal announcement, Kia Starmer has proposed the abolition of NHS England, the management body set up in 2013 under the conservative government. This decisive move aims to improve healthcare management and reduce reported waste, which has been substantial in recent years. Starmer emphasized a return to democratic control, which seeks to free the NHS from bureaucratic constraints and focus on patient care. Critics are concerned that this change could either lead to improved service efficiency or risk the potential dismantling of the NHS. Further, there are ongoing debates regarding financial mismanagement, the use of agency staff, and the handling of sensitive patient data that could arise from these reforms.

Kia Starmer announced the abolition of NHS England, aiming to improve management of the healthcare system that has been criticized for inefficiency over the past 12 years under conservative rule. The decision could either lead to significant improvements in healthcare services or potentially jeopardize the NHS's integrity altogether. The narrative surrounding the wastage of NHS resources is highlighted, including issues of inappropriate prescription practices and high spending on temporary staff, indicating a need for serious reevaluation of management practices within NHS England.

  • 00:05:00 - 00:14:06

The focus now shifts to whether the change will improve care and efficiency without compromising patient data security. Starmer's proposal mentions a transition to Democratic management, hoping to eliminate bureaucratic delays and better direct resources toward patient care. However, concerns about data sharing practices and patient privacy remain prevalent; the challenge lies in establishing trust in how patient data will be managed under new governance. As this transition unfolds, opinions are divided, emphasizing the necessity for public scrutiny and engagement.

well a really gutsy decision by KIA
  • 00:00:02
starmer announced today to abolish NHS
  • 00:00:05
England now before anybody panics and
  • 00:00:08
thinks that this is closing down the NHS
  • 00:00:10
and we lose the NHS that's obviously not
  • 00:00:12
the case NHS England is the management
  • 00:00:15
    body set up under conservative
  • 00:00:17
government in 2013 so over 12 years
  • 00:00:20
we've seen probably not the best
  • 00:00:22
management of our Health Care system
  • 00:00:23
that we could possibly imagine and with
  • 00:00:25
    a spend of200
  • 00:00:28
billion it really doesn't need some good
  • 00:00:30
management so I've got a few thoughts
  • 00:00:32
about that in this video but first of
  • 00:00:34
all if you are new to me I'm Daniel
  • 00:00:35
shens Smith a barrister of England and
  • 00:00:37
Wales and I like to give you my honest
  • 00:00:40
views on these things sometimes and yes
  • 00:00:43
some people don't like personal opinions
  • 00:00:45
but uh if they are personal opinions
  • 00:00:46
I'll make them known but otherwise just
  • 00:00:48
an objective View and saying things that
  • 00:00:50
the mainstream media generally won't say
  • 00:00:53
so please do subscribe to the channel if
  • 00:00:55
    you like this neutral independent
  • 00:00:57
objective approach with a sprinkling of
  • 00:00:59
my own personal opinion now again and
  • 00:01:00
I'll make that clear so what are we
  • 00:01:02
talking about here well K starm among a
  • 00:01:04
lot of different things that he said it
  • 00:01:06
was a it was a long um conference news
  • 00:01:09
conference and among those he said that
  • 00:01:11
they're going to demolish um abolish
  • 00:01:14
demolish frodi and slip uh the NHS
  • 00:01:16
England and bring it back under
  • 00:01:18
    Democratic management now this could
  • 00:01:22
either turn out to be one of the most
  • 00:01:24
genius things that he's ever done and
  • 00:01:27
further down the line we'll look back
  • 00:01:29
and think well now we have a fantastic
  • 00:01:32
Health Care Service that is operating
  • 00:01:34
    smoothly under efficient management
  • 00:01:37
saving money uh not wasting money or it
  • 00:01:41
could be the Catalyst to destroying the
  • 00:01:43
NHS entirely now where do we stand on
  • 00:01:47
this right well let's look at um a lot
  • 00:01:50
of criticism the the NHS has had over
  • 00:01:52
recent Years first of all and uh if we
  • 00:01:55
start going back in time this is not um
  • 00:01:58
out of the blue announcement from Kia
  • 00:02:01
starma there's been lots of more than
  • 00:02:03
hints about this over previous months in
  • 00:02:06
fact if we go back to
  • 00:02:07
October I can show you this press
  • 00:02:09
release Here major Crackdown on NHS
  • 00:02:12
waste this was obviously this new uh
  • 00:02:15
    labor government's approach to healthc
  • 00:02:18
care and a major Crackdown on the waste
  • 00:02:21
in the NHS because actually the the
  • 00:02:24
numbers are quite staggering and now
  • 00:02:25
this is not some people will be pleas a
  • 00:02:27
few of you will be pleased to know this
  • 00:02:29
is not a labor session um but a major
  • 00:02:32
wastage in the NHS all under the
  • 00:02:35
management of NHS England and you'll
  • 00:02:37
recall that I had my own criticisms of
  • 00:02:39
data sharing plans and all of this sort
  • 00:02:41
of stuff which um I'll try and find on L
  • 00:02:43
previous videos but if we look here and
  • 00:02:46
this is going back to this is a Daily
  • 00:02:48
Mail of all articles boot to December
  • 00:02:50
2022 for some context uh so this has
  • 00:02:53
been um the reason for the old article
  • 00:02:55
is to show that this has been ongoing
  • 00:02:57
for some time where the money goes and
  • 00:02:59
where the money is wasted now um they
  • 00:03:01
were talking even then about clinical
  • 00:03:04
negligence uh waste SLS spend of 2
  • 00:03:07
billion pounds and a prescription of
  • 00:03:11
    what would otherwise be over-the-counter
  • 00:03:13
    medicine such as paracetamol paracetamol
  • 00:03:17
if you insist um but the the wastage of
  • 00:03:20
a prescription on something that would
  • 00:03:23
    otherwise be straightforward over- the
  • 00:03:25
    counter stuff is actually quite
  • 00:03:27
    staggering now what's also quite
  • 00:03:29
staggering for a quick little story is
  • 00:03:30
what's happening with my ex mobile phone
  • 00:03:32
provider because they sent me another
  • 00:03:34
bill even though I've already left them
  • 00:03:37
now this is not to be confused with an
  • 00:03:38
overlap bill or a transition bill or
  • 00:03:40
    anything the dates coincided perfectly
  • 00:03:42
and they told me so my contract finished
  • 00:03:44
the day my number moved to the other
  • 00:03:46
company the bill was for another month
  • 00:03:49
and so when I queried it they said well
  • 00:03:50
you'll get a refund at the end of this
  • 00:03:53
month when the final bills produced and
  • 00:03:55
it will discount this month's money and
  • 00:03:57
I said well what's the point in that why
  • 00:03:59
should I pay then to wait for a refund
  • 00:04:01
and he said don't worry about it it will
  • 00:04:02
be refunded and whatever so all the more
  • 00:04:04
reason it was a sensible idea to move to
  • 00:04:06
another network but very shortly I'm
  • 00:04:08
going to be traveling far far from here
  • 00:04:11
so I need reliable internet access and
  • 00:04:14
so I'm going to use S because when I
  • 00:04:18
first had these problems I connected
  • 00:04:19
with s to partner for my channel because
  • 00:04:21
s is an eim data provider for your
  • 00:04:24

buy a plan for your destination and
  • 00:04:29
you'll get SE seamless internet access
  • 00:04:30
on connection when you arrive s Works in
  • 00:04:33
hundreds of countries then you don't
  • 00:04:35
need to wait for a plastic Sim to arrive
  • 00:04:36
in the post you just download it and
  • 00:04:38
install the Sim on your phone or your
  • 00:04:40
device and it will give you seamless
  • 00:04:42
access to the internet and because it's
  • 00:04:44
provided to you by nordvpn you get many
  • 00:04:47
security features that you might expect

  • 00:04:53
might get being in another country when
  • 00:04:55
you access banking apps and things like
  • 00:04:57
that but also you can block intrusive
  • 00:04:59
ads and of course you get web protection
  • 00:05:01
to protect you from malicious websites
  • 00:05:03
malicious downloads which is all the
  • 00:05:06
more important when you go to another
  • 00:05:07
country you're not familiar with the
  • 00:05:08
    destination and you're certainly not
  • 00:05:10
familiar with the Wi-Fi networks or even
  • 00:05:12
any other networks that you might
  • 00:05:13
connect to while you're out there and of
  • 00:05:15

so you can download the app immediately
  • 00:05:24
so it's ready for when you do travel
  • 00:05:26
then when you are traveling you can pick
  • 00:05:27
a plan for your destination pay for it
  • 00:05:30
remembering to use the code black belill
  • 00:05:31
bar to get 15% off the plan and then it
  • 00:05:34

secure access to the internet with the

virtual location so that it will prevent
  • 00:05:42
all these problems you get from being in
  • 00:05:43
another country so I'm definitely going
  • 00:05:44
to use S for seamless internet I'll
  • 00:05:46
report on it when I'm out there check it
  • 00:05:48
out in the description below don't
  • 00:05:49
forget the discount with black belt
  • 00:05:50
barister and thank you to s for
  • 00:05:52
sponsoring this video so for instance
  • 00:05:53
people are coming in with aches and
  • 00:05:54
    pains and being prescribed paracetamol
  • 00:05:57
which is available obviously over the
  • 00:05:59
counter within certain age limits and
  • 00:06:01
purchase limits and so on this is not
  • 00:06:03
medical advice you understand um but
  • 00:06:05
    they've been prescribed paracetamol for
  • 00:06:07
general aches and pains and that has
  • 00:06:09
cost the Health Service 74 million and
  • 00:06:13
this is just the the statistic for 2021
  • 00:06:15
    to
  • 00:06:16
2022 um not only that um on completely
  • 00:06:19
separate things gas and electricity it
  • 00:06:21
    was researched and reported the
  • 00:06:23
taxpayers allian analyzed that for uh
  • 00:06:26
2019 to 2020 they were wasting huge
  • 00:06:31
amounts of money and wasting nearly 220
  • 00:06:35
million pound a year on overpriced gas
  • 00:06:37
and electricity fast forward to December
  • 00:06:40
2023 so a bit of a timeline going here
  • 00:06:42
and this is just the last few years it
  • 00:06:45
was reported in the BBC uh the NHS spent
  • 00:06:47
    more than
  • 00:06:49
£384 million on agency staff in the
  • 00:06:53
north west of England over the last 3
  • 00:06:55
years all according to statistics from
  • 00:06:57
the rural College of Nursing so why is
  • 00:06:58
that important it may not be obvious to
  • 00:07:00
you that agency staff is obviously going
  • 00:07:02
to be more expensive than full-time
  • 00:07:04
staff so Ergo there is um a lack of
  • 00:07:07
management of the staff that they
  • 00:07:09
actually need and so to plug shortages
  • 00:07:13
of Staff because either they haven't
  • 00:07:16
planned for the staff they need or they
  • 00:07:18
get what they call a regrettable
  • 00:07:20
turnover which is u a churning over
  • 00:07:23
staff leaving and staff coming in and
  • 00:07:25
there's a gap in between and so they
  • 00:07:26
need agency staff to fill those gaps and
  • 00:07:29
potentially they need a huge amount of
  • 00:07:30
money £384 million for that for those
  • 00:07:35
three years uh to plug these gaps of
  • 00:07:38
staff and this was criticized as a false
  • 00:07:41
economy because they could have employed
  • 00:07:42
over 4,000 full-time staff instead of
  • 00:07:46
employing these agency staff now the
  • 00:07:48
Crackdown on wasted expenditure was uh
  • 00:07:51
announced by this government in October
  • 00:07:54
2024 and included things such as waste
  • 00:07:58
of disposable medical devices and single
  • 00:08:03
use items which aside from the cost
  • 00:08:05
    alone was contributing to
  • 00:08:08
156,000 tons of clinical waste produced
  • 00:08:11
every year alone other examples quoted
  • 00:08:14
included the mid Yorkshire trust using
  • 00:08:17
    330,000 single-use torices in a year but
  • 00:08:21
a single reusable Toray can be used
  • 00:08:24
    10,000 times in a one-year trial
  • 00:08:27
    reusable Alternatives save £20,000 in
  • 00:08:29
procurement and that's just for one
  • 00:08:31
    simple item in Northampton Hospital's
  • 00:08:34
    NHS trust a single Opthalmology
  • 00:08:36
    Department saved 1,000 pair of
  • 00:08:37
disposable scissors and £12,000 in one
  • 00:08:40
year just by switching to reusable pairs
  • 00:08:43
of scissors these are the kind of things
  • 00:08:45
that we're talking about here using
  • 00:08:47
reusable items as opposed to single use
  • 00:08:50
items and saving thousands of pounds of
  • 00:08:52
money now I've had people within the NHS
  • 00:08:54
talking to me recently and you might be
  • 00:08:57
aware that certain people get criticized
  • 00:08:59
for whistleblowing and things like that
  • 00:09:01
but I've had people talk to me telling
  • 00:09:03
me that they have thousands of pounds
  • 00:09:06
wastage almost daily in their everyday
  • 00:09:09
work because somebody might open the
  • 00:09:12
wrong item but because it's a very
  • 00:09:14
expensive single value item and single
  • 00:09:17
use item that is then just going
  • 00:09:19
straight in the bin because it was the
  • 00:09:21
wrong thing and when you multiply this
  • 00:09:23
across thousands and thousands of items
  • 00:09:25
across all the trusts across the country
  • 00:09:28
it is quite obvious that how much of
  • 00:09:30
this money can then get wasted now it
  • 00:09:32
will be interesting to see whether there
  • 00:09:34
    is cross-party collaboration and
  • 00:09:36
agreement on this because as I said at
  • 00:09:38
the outset and it's not just
  • 00:09:40
scaremongering this could either be the
  • 00:09:42
most genius plan moving forward because
  • 00:09:44
I know a lot of people within the NHS
  • 00:09:46
say that the management of the NHS is
  • 00:09:49
just not up to scratch and it really
  • 00:09:51
needs to be revamped or it could be the
  • 00:09:54
thing that leads to the destruction of
  • 00:09:56
the NHS breaking it apart selling it off
  • 00:09:59
uh privatizing certain areas of it and
  • 00:10:01
the reality is something we will only
  • 00:10:03
discover much later on either when it's
  • 00:10:05
worked or when it's too late to do
  • 00:10:07
anything about it but the real question
  • 00:10:09
here is probably one of control and as
  • 00:10:13
with anything that is under somebody's
  • 00:10:15
control you need to ask yourself do you
  • 00:10:18
trust the people in control now to be
  • 00:10:21
fair I know a lot of people do trust
  • 00:10:23
that this is a sensible Way Forward a
  • 00:10:25
sensible decision and it's under strong
  • 00:10:27
leadership and so on but equally I
  • 00:10:30
understand that there are a lot of
  • 00:10:31
people that are very concerned and don't
  • 00:10:33
think that this is under strong
  • 00:10:34
leadership and don't think that this is
  • 00:10:37
proper management now Kama today uh was
  • 00:10:40
saying that he's bringing the management
  • 00:10:43
of the NHS as a whole back into

Democratic control so that should really
  • 00:10:49
mean that there are different voices
  • 00:10:50
heard there are different ideas heard
  • 00:10:52
they are debated properly and ultimately
  • 00:10:54
it is a fair and well-managed and
  • 00:10:56
balanced system he said that this move
  • 00:10:59
will put the NHS back at the heart of
  • 00:11:01
government where it belongs he says
  • 00:11:04
freeing it to focus on patience less
  • 00:11:06
bureaucracy with more money for nurses
  • 00:11:09
he says that the NHS will refocus on
  • 00:11:11
cutting waiting times in your hospital
  • 00:11:14
you the viewer wherever you live but
  • 00:11:16
another very important point is that
  • 00:11:17
because NHS is a devolved matter
  • 00:11:20
obviously this can only pertain to
  • 00:11:22
England because as the name suggests NHS
  • 00:11:25
England is responsible for England and
  • 00:11:28
the it'll be separate matter for
  • 00:11:30
Scotland Wales and Northern Ireland and
  • 00:11:33
finally one question I would have if it
  • 00:11:35
were posed to government is what about
  • 00:11:38
patient data because you you might
  • 00:11:40
remember from nearly 3 years ago now uh
  • 00:11:43
I did a video on NHS digital which has
  • 00:11:46
now been merged with NHS England and
  • 00:11:48
they were making your data available to
  • 00:11:51
third parties and this was an opt out as
  • 00:11:54
opposed to an opt in so you had to go
  • 00:11:57
through the process of opting out to
  • 00:11:58
prevent them for sharing your data
  • 00:12:01
rather than opt in to allow them to
  • 00:12:02
share your data now there were
  • 00:12:05
    counterarguments to that because
  • 00:12:07
obviously to do research and to find new
  • 00:12:10
cures and things like that they do need
  • 00:12:12
data to work with but at the same time
  • 00:12:14
the data was what they called
  • 00:12:16
    pseudonymized rather than anonymized so
  • 00:12:19
with some work you could trace the
  • 00:12:21
information back to the original person
  • 00:12:23
so it wasn't entirely Anonymous so there
  • 00:12:25
were criticisms with that so there were
  • 00:12:26
counterarguments to it but a lot of
  • 00:12:28
people that they didn't want their data
  • 00:12:30
    shared with third parties which
  • 00:12:31
ultimately might end up the data might
  • 00:12:33
be sold to other parties and this is a
  • 00:12:36
concern that I have here because if the
  • 00:12:38
NHS management is brought back under
  • 00:12:41
what they call Democratic control which
  • 00:12:43
is back into government how do we know
  • 00:12:45
and how can can we be assured that
  • 00:12:47
decisions made for the NHS the
  • 00:12:49
management of the NHS and ultimately you
  • 00:12:51
and your data your patient data and what
  • 00:12:53
what's going on with you how can we be
  • 00:12:55
sure that that is always going to be in
  • 00:12:58
your interest and is not going to be
  • 00:13:00
sort of sliced and carved up in order to
  • 00:13:02
sell it on to third parties that is a
  • 00:13:04
concern that I would have and something
  • 00:13:06
that certainly needs to be monitored to
  • 00:13:09
make sure that doesn't happen to the
  • 00:13:10
detriment of the general public that is
  • 00:13:12
certainly something I'll be keeping my
  • 00:13:14
eye on so for that alone please
  • 00:13:15
subscribe to the channel because I will
  • 00:13:17
come back and talk to you about that if
  • 00:13:18
I find anything going on about that but
  • 00:13:20
I hope that is a broad overview of
  • 00:13:22
what's going on the NHS isn't suddenly
  • 00:13:24
disappearing as I think at least some
  • 00:13:26
people will probably think um but
  • 00:13:27
certainly the management of it it is
  • 00:13:30
going to be abolished and brought back
  • 00:13:32
under government which is either a
  • 00:13:34
genius thing or a recipe for disaster
  • 00:13:36
let me know which side of the fence you
  • 00:13:37
fall there I think this is a very very
  • 00:13:40
difficult one to call let me know your
  • 00:13:42
thoughts in the comments below if you've
  • 00:13:43
had experience you can email me
  • 00:13:44
anonymously I will keep your anonymity
  • 00:13:47
absolutely but share any messages that
  • 00:13:49
you might have if you wish me to let me
  • 00:13:51
know what your position is what your
  • 00:13:53
views are if you're working particularly
  • 00:13:55
if you're working within the NHS let me
  • 00:13:57
know how it is for you I know some
  • 00:13:58
people have done already um you know who
  • 00:14:00
you are and so let me know what you
  • 00:14:02
think do subscribe to the channel thank
