00:00:02
So what if the secret to younger looking
skin has been hiding in plain sight, but
00:00:09
no one's talking about it in this video,
I'm revealing the surprising truth behind
00:00:14
estrogen creams, the anti aging benefits
that you could see that would change
00:00:19
everything for your skin care routine.
00:00:22
And the risks you need
to know before trying it.
00:00:26
Trust me, you won't
want to miss this video.
00:00:28
I had a patient come in yesterday
and she said, doc, it's gone.
00:00:33
Those wrinkles are gone.
00:00:35
She pointed to the smile lines
right here at the side of the face.
00:00:38
I said, great, you just
saved yourself 5, 000.
00:00:43
But what did you do?
00:00:45
You see, it took her two
months to get in and see me.
00:00:48
And in the meantime, she
started something, a cream.
00:00:52
I go, well, what?
00:00:53
And she was a little vague.
00:00:54
She goes, well, you know,
a cream for down there.
00:00:58
And I used it up here.
00:01:00
But it's not going to be
that simple for all of you.
00:01:03
So let's dive in.
00:01:04
And by the way, I'm wearing glasses today.
00:01:07
I had lenses placed in my eyes
and we're still adjusting them.
00:01:11
I can't be exposed to certain frequencies
of, or wavelengths, sorry, of light.
00:01:15
So bear with me.
00:01:17
Now let's start with a quiz, a
quiz you cannot possibly get wrong.
00:01:21
Here it is.
00:01:22
What percent of women get estrogen loss?
00:01:26
So what percent of you who are women
will get estrogen loss in your lifetime?
00:01:33
100 percent exactly.
00:01:35
It's not if it's when that's
the importance of this video.
00:01:40
This is a nice chart
showing estrogen levels.
00:01:43
You can see.
00:01:44
By age 35, it's kind of peaked.
00:01:46
Uh, a little erroneous here.
00:01:47
By 40 or 50, there's much less
than what they're showing.
00:01:51
The estrogen is shown in the dark purple.
00:01:54
But you can see clearly, 60, 70,
80, uh, the levels are very low.
00:01:59
In that, um, light purple kind of color.
00:02:03
I'm sure you'll also ask, well, if we're
low on thyroid, we just add more thyroid.
00:02:08
Why don't we add more estrogen?
00:02:10
I'll discuss.
00:02:11
And before we really get into it, yes,
I'm talking hormones today, specifically.
00:02:17
Topically, of course, but the loss of
estrogen on the skin in the body and
00:02:22
fluctuations of hormones, I see in my
clinic starting younger and younger.
00:02:26
And if it's starting around
35, that's not too uncommon.
00:02:30
And don't be gaslit by the way, by
your doctor, if you're presenting
00:02:34
with kind of systemic symptoms of odd
things, things you might not associate
00:02:39
with like paresthesias, where you
just feel differently sensations
00:02:43
throughout your body, frozen shoulder.
00:02:47
Bloating, thinning of the
hair, distraction, brain fog.
00:02:51
So definitely see an expert, but I want
to get that out of the way first, because
00:02:55
many of you are suffering from this.
00:02:57
I see it in my clinic.
00:02:58
And in fact, eight of the last 10 patients
I had this week in my clinic present
00:03:03
newly, they were all perimenopausally.
00:03:06
So this is all related.
00:03:08
It's a time of change for women.
00:03:11
Check this out.
00:03:12
In one study after six months.
00:03:14
Of topical estrogen use, skin
elasticity and firmness had
00:03:19
improved in wrinkle depth.
00:03:21
Are you ready?
00:03:22
And pore size had decreased
by 61 to 100 percent.
00:03:30
Colossal.
00:03:31
So on to the cream for down there.
00:03:34
Used up here.
00:03:36
Now, before you rush out and smear
your face with hormones, let's get
00:03:40
into the science a little bit, the
pros and the cons and whether it's
00:03:44
a good idea to swap your current
moisturizer for an estrogen boosting one.
00:03:50
Okay, here is the exact
cream that she brought in.
00:03:56
Estradiol Vaginal Cream 0.
00:03:58
01%.
00:03:59
Now, it is estradiol, and there are
other ingredients in here, but nothing
00:04:04
that would make, uh, such a big change
that all the wrinkles were gone.
00:04:09
But this is exactly
what you're looking for.
00:04:11
And it was, uh, I think
she had about 40 U.
00:04:14
S.
00:04:15
dollars.
00:04:15
Dollars.
00:04:16
You S T spoiler alert here.
00:04:20
It's more complicated than you think.
00:04:22
We're talking hormones.
00:04:24
So it's complex by nature, but the most
commonly used and cheapest estradiol
00:04:30
cream that some women might use, what
we call off label for facial application
00:04:36
is estrase, which is estradiol 0.
00:04:40
01%.
00:04:42
Estrase is a prescription cream primarily
intended for vaginal atrophy and menopause
00:04:48
and those related symptoms, but it's
repurposed for skin application due to
00:04:54
estrogen's potential anti aging benefits.
00:04:58
We'll talk about compounded
versions in a minute because some
00:05:01
of you are using those already.
00:05:04
Uh, but if you're on hormone replacement
therapy, Do you still need the creams?
00:05:09
That's the most asked question.
00:05:12
And the answer is yes.
00:05:13
In my practice, even those who are on
hormone replacement patches, some people
00:05:19
are on pellets, some people put creams
on the arms that are absorbed into the
00:05:23
skin or different parts of the body,
they still benefit from the cream.
00:05:26
From the cream that's placed
directly on the skin, where you
00:05:30
want to affect change for wrinkles
and to help thicken your dermis.
00:05:35
However, S trace is
typically not marketed.
00:05:38
You won't see it marketed officially
recommended for facial use and
00:05:42
its price can vary, of course.
00:05:44
And some people have it
covered by insurance.
00:05:47
I know some of you comment below,
and then there are generic versions.
00:05:51
Of estradiol creams, which
are also available that you
00:05:54
can get typically cheaper.
00:05:57
And then of course
there's compounded ones.
00:05:59
Now this gets complex.
00:06:00
Many of you are on these because
these are the least expensive.
00:06:04
That means that a pharmacy that
does compounding makes it up.
00:06:08
It's usually not exactly the
same dosing as the ones that
00:06:11
are commercially available.
00:06:12
And often they'll add a few other elements
or ingredients and it's much cheaper.
00:06:17
Here's the hook.
00:06:19
Uh, and I get compounded.
00:06:22
You need to know those of us who use a
medications like this, you need to know
00:06:26
which pharmacies doing it now, when you're
talking hormone replacement, we know,
00:06:31
because what we'll do is put people on
hormones and then you measure levels and
00:06:36
in when you're seeing tens and hundreds
of people, you then know, oh, we're This
00:06:41
dosing from this pharmacy works like this.
00:06:44
Um, and you'll go online.
00:06:45
If you search compounding pharmacies,
you'll see a lot of nefarious
00:06:49
issues that have gone on in the
past and a non sterility issues.
00:06:53
So know where you get it.
00:06:54
If you're getting compound aversions,
I do want to make that point.
00:06:58
These are not regulated pharmacies and you
know, the state in America, the state with
00:07:03
our workers, people are making mistakes.
00:07:06
Rampantly.
00:07:07
So know where you're getting your product.
00:07:09
Now, melasma, another opt ask question.
00:07:13
These are the dark spots
called the mask of pregnancy.
00:07:16
And the reason is a lot of people have
melasma from the sun, from heating,
00:07:22
And some people get them for hormones.
00:07:24
So can you get that with this?
00:07:25
Yes, you have to be mindful if you're
melasma prone in a moment, I'll give
00:07:31
you a tip and a trick for those of you
suffering from dark spots from hormones.
00:07:36
See if you can use it or not.
00:07:39
Before you reach for your estrogen
cream, the elephant in the room remains.
00:07:43
We all know that aging
isn't just about birthdays.
00:07:47
It's about sun exposure, genetics,
diet, and all of those fun
00:07:51
lifestyle factors that play a role.
00:07:54
Air pollution is And
pollution damage to your skin.
00:07:57
So the number one ager of
your skin is photo damage.
00:08:01
So before you go and go directly
for the estrogen cream, you want
00:08:05
to take care of your basics first.
00:08:07
You want to think of estrogen as the
skincare fairy who keeps your skin firm.
00:08:13
Plump and hydrated as menopause
hits, that fairy goes on vacation
00:08:18
and suddenly your skin isn't
bouncing back the way it used to.
00:08:23
I shouldn't say on vacation.
00:08:24
I should say on retirement because
it rarely goes the other way around.
00:08:29
You see, you lose collagen
up to about 30 percent in the
00:08:32
first five years of menopause.
00:08:35
Think of a last thing that weakens.
00:08:37
Skin hydration plummets.
00:08:39
The result is wrinkles and dryness.
00:08:42
Yep.
00:08:42
I did say dryness.
00:08:44
Many of you relate to that.
00:08:45
And that of course leads to sagging.
00:08:48
So why did my patient get
such an amazing result?
00:08:51
Why is estrogen cream the
holy grail for so many people?
00:08:55
Well, it's like the
skin's personal trainer.
00:08:58
It keeps everything in shape.
00:09:00
It helps produce collagen.
00:09:02
It helps produce elastin and it helps
those magical molecules that hold onto
00:09:07
water in your skin that some of you try
and take topically like hyaluronic acid.
00:09:12
Without it, the skin
becomes less firm and dry.
00:09:17
These benefits are proven by science.
00:09:20
This is why during menopause, skin
can become fragile and slower to heal.
00:09:25
Remember, we always look at
the skin's ability to heal.
00:09:29
And correlate that with its healthiness.
00:09:30
And if things make it heal quicker,
we know that's great skincare.
00:09:34
One thing you get is increased blood flow.
00:09:37
We know how much that helps
is why we like Gua Sha.
00:09:40
But remember when you get increased blood
flow, some people will see increased.
00:09:44
Broken capillaries, increased
light redness of the skin.
00:09:49
So be mindful for that.
00:09:50
So then can you just slather on the
estrogen cream and reverse aging?
00:09:55
It's not quite that simple,
however, and here's why.
00:09:58
Think of your estrogen cream, kind
of like fertilizing your garden.
00:10:02
Okay.
00:10:02
You add nutrients to make
sure the plant grows better.
00:10:06
And this would be your wrinkles
going away, but you overdo it.
00:10:10
In your garden and you're going to
get weeds, but worse yet, you can
00:10:14
accidentally fertilize your neighbor's
yard in the same way using estrogen
00:10:19
cream can have effects beyond your skin.
00:10:22
And that's what we're worried about.
00:10:24
You see, you also have breasts.
00:10:27
possibly a uterus, and you can
even extend that to ovaries.
00:10:31
There's evidence that unopposed estrogen
can affect the ovaries, and ovarian cancer
00:10:36
is something you definitely don't want.
00:10:38
So there are risks and side effects.
00:10:41
So this is where things get tricky while
topical estrogen is a game changer for
00:10:45
some understand there is some evidence
that it gets absorbed into the body and
00:10:51
potentially going to cause side effects.
00:10:54
Where do we get our data
from face absorption from?
00:10:57
Because there isn't much if any.
00:10:59
Well, it's from vaginal application and
the fact that and there is division here.
00:11:05
A lot of people think that, okay, you get.
00:11:07
systemic absorption from
vaginal application.
00:11:11
And I believe that's true because enough
of my patients will say if they're
00:11:15
on vaginal hormone replacement, so
vaginal esters, estradiol, uh, they get
00:11:22
tenderness in the breast for a while.
00:11:25
And, but we look at the studies from
that application and really the risk
00:11:29
of uterine cancer and breast cancer.
00:11:33
Is not appreciably increased, although
some people would disagree with
00:11:38
that, but by and large, the use of
vaginal estrogen cream is deemed safe.
00:11:44
Thus, we extrapolate that on the face.
00:11:46
It's deemed safe.
00:11:47
The levels aren't high enough to cause
those problems, but I have to tell you
00:11:51
in clinical practice vaginally, which
there's much more absorption than on
00:11:55
the facial skin, because it's a mucous
membrane, people do get some systemic.
00:12:01
Symptoms just doesn't bear out
that they get the, uh, side
00:12:06
effects of increased cancer risk.
00:12:09
Uh, that's appreciable.
00:12:10
Now, here's from breastcancer.
00:12:13
org vaginal estrogen safe
for women with breast cancer.
00:12:18
These are certainly worth
reading on your own.
00:12:20
If you want more information,
are you getting screened?
00:12:23
Are you getting mammography?
00:12:25
Low dose radiation mammography
better yet for many people.
00:12:29
MRI.
00:12:30
And yes, I know we have the
nurses health initiative trial.
00:12:33
I did a whole video on that.
00:12:34
I'll link that at the end.
00:12:35
It's important for you to know.
00:12:37
And I'm a believer in hormone
replacement therapy for women, but
00:12:41
you have to be the right candidate.
00:12:42
And I think it's worthwhile,
of course, going over with your
00:12:46
provider, your risk factors, if
you're going on a cream like this.
00:12:50
And yes, the nurses health initiative
study, it was a flawed study.
00:12:55
Think progestins that's for another video.
00:12:58
I did make that video.
00:12:59
And again, I'll link it.
00:13:00
it.
00:13:01
So let's go through the exact risks.
00:13:03
I just mentioned breast
tissue stimulation.
00:13:07
If you have a history or personal
history of breast cancer or you're at
00:13:10
high risk, then it can be a concern.
00:13:13
Definitely talk to your doctor.
00:13:14
Well, now you're going to ask,
what about blood clots and
00:13:17
cardiovascular risks from the data?
00:13:20
Everything looks safe.
00:13:21
I feel comfortable here for most people,
but systemic absorption could still
00:13:26
lead to issues like blood clotting.
00:13:29
Uh, especially with long term use
again, see your provider, but for
00:13:33
most people, depending on the risk
factors, it's very reasonable risk.
00:13:38
If any risk here to take skin
irritation, like any cream,
00:13:42
I sometimes see some redness.
00:13:43
We talked about broken
capillaries already.
00:13:46
I talked about melasma, which you're
going to come back to fragility
00:13:50
of vessels in the face for sure.
00:13:53
One thing, if you have melasma.
00:13:55
And particularly maybe you're on the
birth control pill and then you got
00:13:59
that mask of pregnancy or the mustache.
00:14:01
You want to consider using it in
an inconspicuous area, like maybe
00:14:05
the skin at the lateral face here.
00:14:08
Uh, maybe where, if you're wearing
your bangs or hair down, maybe, uh, on
00:14:12
top of your forehead, small amounts,
go slowly and then start slowly.
00:14:18
If you're going to start the
cream and then avoid being
00:14:20
exposed to heat or sun that can
be confounding variables for this.
00:14:24
Remember, just like everything
I talk about, what works wonders
00:14:28
for your neighbor means it
could be a disaster for you.
00:14:32
And that's why you're seeing a
provider with things like these.
00:14:35
Luckily too, we do have alternatives.
00:14:38
So if using estrogen on your skin
sounds a little too risky for you or
00:14:43
you have risk factors, don't worry.
00:14:45
There are alternatives.
00:14:46
Let's talk about phytoestrogens
and M E A P for a minute.
00:14:52
Phytoestrogens are plant based compounds.
00:14:54
You would have heard of these.
00:14:56
They mimic estrogen, but
without the full hormonal punch.
00:15:00
What did the study say?
00:15:01
Well, they suggest that these can
improve your collagen production, improve
00:15:05
wrinkles, and increase skin hydration.
00:15:08
And it's not by taking estradiol.
00:15:10
It's these phytoestrogens.
00:15:12
They also too, they are antioxidants.
00:15:16
Genistein is a naturally
occurring compound abundantly
00:15:19
found in these soybeans.
00:15:21
Fava beans and other lentils.
00:15:24
Yes.
00:15:24
The chemical structure is similar
to estradiol, but it's a plant based
00:15:29
compound that mimics your estrogen.
00:15:31
So you can look for products
that have that in it.
00:15:34
The other one is M E A P
methyl estradiol propanoid.
00:15:40
Say that a few times.
00:15:41
That's a tough one.
00:15:42
But it's another option.
00:15:43
It binds estrogen receptors on the skin
but it does not get absorbed into the
00:15:48
bloodstream, meaning fewer systemic risks.
00:15:52
Systemic means throughout your body.
00:15:54
So think of it, uh, as an estrogen light.
00:15:57
So these products, there are some
on the market that have these.
00:16:01
Often they're really expensive.
00:16:02
Like, I know one, it's about
400 for to get started on this.
00:16:07
So These are patented ingredients.
00:16:09
So if you're going to go down that road,
you're going to have to shell out the
00:16:12
safest and most proven thing you can do
for your skin, of course, is avoiding,
00:16:17
um, the sun damage, the photo damage.
00:16:20
So using sunscreen, retinoids,
glycolics, and using peel pads.
00:16:25
If you're on those products, you're
doing 80 to 90 percent of the work.
00:16:29
A great thing to add to that routine
is red light and red light therapy.
00:16:33
And there's a complete
video up here on that.
00:16:35
And one down here on peel pads that
you can use at home, glycolic ones
00:16:40
to help with your sunspots, keep your
skin smooth, to keep your skin and
00:16:43
body beautiful, healthy, and vibrant.
