    look at and so when we look at the brain

    divide it into regions and we'll try to

    do that today and and talk about what

    area that we we often think of when we

    talk about the brain uh but there

    clearly are some very important deep

    and it does have what at first glance

    unique uh characteristic pattern here

    that does allow us to identify different

    areas and the first level of dividing

    the brain into regions is dividing it

    into lobes so as I'm sure we all

    remember from school the frontal lobe

    and the parietal lobe are separated by

    the Central sulcus and that's the main

    sulcus that separates these two regions

    of the brain there's a lateral or Sylvan

    fissure that separates the frontal lobe

    from the temporal lobe there's an

    transverse fissure which will divide the

    and then now the cranial nerves and the

    more evident so a lot of complicated

    anatomy and there's a long history of

    trying to understand the anatomy of the

    brain and and one of the earliest uh

    important steps in trying to understand

    the cortex since it did seem like a very
  • 00:05:05
    random uh structure was some of the work
  • 00:05:08
    that broadman did so broadman was a
  • 00:05:10
    German neurologist not a neurosurgeon
  • 00:05:12
    unfortunately but still very very smart
  • 00:05:14
    um and uh he was one of the first to
  • 00:05:18
    really look at these sub regions and he
  • 00:05:20
    looked at them histologically looked at
  • 00:05:21
    the cell types in those areas and began
  • 00:05:24
    to divide the brain into regions and so

    this is a colorized version of the

    original diagram that brodman published

    it was done in black and white dividing

    this has been refined quite a bit over

    brain and to think about an area that is

    prefrontal which is the pink area here

    and we can talk about the prefrontal

    cortex and what happens there we can

    talk about the motor region of the
  • 00:06:25
    frontal lobe so we can divide this

    frontal lobe into two regions we can

    area which is the blue and we have a

    primary sensory area here we also have

    another primary sensory area in the back

    of the brain and then a lot of the light

    association areas are very interesting

    because a lot of higher order

    so we'll start with the prefrontal

    cortex this is the part of the brain

    that is the most complex and most

    developed in humans and so this really

    is is really the closest thing to where

    our mind sits if you will a lot of
  • 00:07:12
    complex Behavior cognitive behavior

    social behavior personality expression
  • 00:07:17
    those of you that have taken care of
  • 00:07:18
    patients with prefrontal lobe injuries
  • 00:07:21
    know that that people change when this
  • 00:07:23
    part of the brain is not working well so
  • 00:07:26
    a lot of executive functions as well lie
  • 00:07:28
    in here so a lot of our high High
  • 00:07:29
    cognitive processing trying to follow
  • 00:07:32
    rules perform multiple tasks at a time
  • 00:07:34
    control impulses and again a lot of
  • 00:07:37
    Personality lies in the prefrontal
  • 00:07:39
    cortex and a lot of working memory is
  • 00:07:42
    here too so for those of us that have
  • 00:07:44
    had patients that have a hard time with
  • 00:07:45
    working memory hanging on to a rule uh
  • 00:07:48
    hanging on to short-term memory and
  • 00:07:50
    being able to to retrieve it while doing
  • 00:07:52
    a task it's very difficult when you have
  • 00:07:54
    a prefrontal cortex
  • 00:07:56
    injury um when patients have a head
  • 00:07:58
    injury for example which is one of the
  • 00:08:00
    most common causes for brain injury uh
  • 00:08:02
    this this prefrontal cortex especially
  • 00:08:04
    the frontal lobe and the orbital frontal
  • 00:08:06
    surface are very prone to moving within
  • 00:08:08
    the cranial Vault and getting damage so
  • 00:08:09
    that's why we we see quite a bit of that
  • 00:08:12
    so that's a brief brief kind of idea of
  • 00:08:15
    what happens in the prefrontal area if
  • 00:08:17
    we move back to this darker right area
  • 00:08:19
    this is the the more posterior portion
  • 00:08:21
    of the frontal lobe and this is the
  • 00:08:23
    motor area and so the motor area has
  • 00:08:25
    multiple regions there's a primary motor
  • 00:08:28
    cortex which is broadman area four and
  • 00:08:30
    then there premotor areas there's an i
  • 00:08:32
    field and a a language field here so the
  • 00:08:35
    primary motor cortex is the one we all
  • 00:08:37
    learned back in school this is the
  • 00:08:39
    precentral gyus so it's right in front
  • 00:08:41
    of that Central gyus so it's right at
  • 00:08:43
    the very back of the frontal lobe very
  • 00:08:45
    large paramal cells here which is why
  • 00:08:47
    broadman identified this as region four
  • 00:08:49
    it's a very distinct anatomic region and
  • 00:08:52
    it does have a somatotopic organization
  • 00:08:54
    so a lot of us remember the homunculus
  • 00:08:56
    this idea of a little map of the body
  • 00:08:59
    within the frontal lobe so the more
  • 00:09:01
    lateral parts of this are areas that are
  • 00:09:03
    controlling movement of hand and face
  • 00:09:05
    and mouth the more medial and Superior
  • 00:09:08
    areas are where the leg area is located
  • 00:09:10
    and this becomes important in patients
  • 00:09:11
    that have a stroke or a tumor where out
  • 00:09:15
    here you're going to expect more facial
  • 00:09:16
    droop hand
  • 00:09:18
    weakness uh an ACA stroke for example
  • 00:09:21
    which would affect more the parentral
  • 00:09:22
    lobu may result in just leg weakness but
  • 00:09:24
    the arm and hand and face may be okay so
  • 00:09:26
    this this organization is very important
  • 00:09:29
    function
  • 00:09:31
    and the motor cortex is is a very simple
  • 00:09:36
    and starting basic starting point for
  • 00:09:38
    for motor movement but there's a lot of
  • 00:09:40
    motor planning that's involved and
  • 00:09:42
    that's where the premotor area comes
  • 00:09:43
    into play so as you get closer to that
  • 00:09:45
    prefrontal area where you're more uh
  • 00:09:48
    cognitively engaged that's also where
  • 00:09:50
    you become uh where you're focusing your
  • 00:09:53
    planning and your access to motor plans
  • 00:09:56
    and selecting different motor plans and
  • 00:09:58
    inhibiting competing plan so a lot of
  • 00:10:00
    that happens here in this premotor
  • 00:10:02
    cortex area which is a very important
  • 00:10:04
    area and as as you know if you've had a
  • 00:10:06
    patient that's at a premotor uh cortex
  • 00:10:08
    injury a lot of times they may even look
  • 00:10:10
    like someone that has a complete motor
  • 00:10:12
    paralysis because they just have a very
  • 00:10:14
    hard time with initiating and having
  • 00:10:16
    that drive to move uh moving the eyes is
  • 00:10:19
    a very complex motor function so that
  • 00:10:20
    occurs in the in the frontal eye field
  • 00:10:22
    which is located right here and then
  • 00:10:25
    broka area which is uh this area here uh
  • 00:10:28
    is a speech area and it's a pretty large
  • 00:10:30
    area because speech production can
  • 00:10:32
    happen with your mouth it can also
  • 00:10:34
    happen with your hand you can sign
  • 00:10:36
    speech you can write and you can speak
  • 00:10:38
    and so broka area is a very complex area
  • 00:10:41
    it's not a coincidence that it's located
  • 00:10:43
    right next to the part of the homunculus

    where your mouth and hand is right so it

    makes sense that an area that's going to

    initiate that type of motor plan is

    understanding stimuli as they come in

    and a lot of it happens in this primary

    sensory cortex or this post Central

    gyrus a gyrus right behind the Central

    sulkus and the primary seat sensory

    cortex is very similar to the motor

    cortex in that it also has a a

    somatotopic organization so there is a

    similar organization where the medial

    portion is the leg the more leral

    portion is the hand and face and again

    the the distribution here is is biased

    towards areas like the fingers the hand

    nice dominant sulcus right in the middle

    and this is area 17 and this is primary

    visual cortex so the visual input that

    comes from the eyes relays through the

    thalamus and comes back to the occipital

    idea where you have a primary area so

    this is an area that's going to detect a

    flash of light for example in a certain

    forward to the association areas in two

    different streams there's a dorsal

    stream which actually goes towards a

    parietal lobe and that's a part of the

    brain that that is focused more on on

    where something is where an object is uh

    where you are geographically where

    structures are within your world uh and

    then there's a ventral stream that goes

    towards a temporal lobe which is an

    association area that's more of a what

    area so a lot of our recognition of a

    face of an object naming things is going

    to occur here so one example of how

    input coming to the primary visual

    cortex might make you blind cortically

    blind if you weren't able to get that

    tracks are absolutely critical and many

    dramatic dysfunction even though the

    cortical regions are working okay so if

    the connections are not there uh you

    know this complex computer falls apart

    so it's very important to keep in mind

    cortex is fine but movement is still

    affected because of these basil ganglia

    these deep structures so having just a

    normal motor C and intact white matter

    tracks doesn't mean the motor function

    hypothalamus like with a tumor like a

    cranial foma they can become severely

    obese because they they have total
